The Fertility Mission

My wife and I are trying to conceive. She has a lower egg count, so we're heading down the IVF path (ICSI, they inject the sperm directly into the egg). We're planning to freeze and bank embryos first, then implant.

My job is simple: make the best swimmers possible.

Long term I also want to keep running HCG even when I eventually go back on cycle to see if it actually helps maintain fertility while on, like the literature suggests. There's a lot of talk about it but not many real world logs documenting it so I want to be that guy.

PCT and Recovery Timeline
  • Mar 2025: Came off everything cold turkey post-comp
  • Apr to Jul 2025: PCT with Clomid 50mg + Nolva 20mg + HCG 1000IU EOD + HMG 75IU EOD
  • Sep 2025: Back on TRT (Test E 40mg 2x/week), life without test was rough after month 4-5
  • Nov 2025: Added Exemestane 6.25mg 2x/week for E2 control
  • Jan 2026: Added HCG 500IU 4x/week + HMG 75IU (fertility protocol begins properly)
  • Mar 2026: Replaced HMG with Puregon 75IU 4x/week (ran out of HMG)

Lessons Learnt
  • Test + Clomiphene together doesn't work, learnt that the hard way, they cancel each other out
  • HCG mimics LH but doesn't show up as LH on blood tests, different molecules
  • Only ran HMG for one month initially which wasn't enough, sperm count dropped off after ~5 months. Committing to a longer protocol this time
  • HMG ran out so switched to Puregon (recombinant FSH), bumped dose from 37.5IU to 75IU to compensate

Concentration dropped hard after stopping HMG but motility has been climbing steadily which is encouraging. The ICSI route means we don't need massive numbers, we just need good quality swimmers.

Training: Upper/Lower 4-Day Hypertrophy Split

Just switched from PPL to an Upper/Lower split (8-week block):
  • Mon, Upper A (compound focus): Smith Decline Bench 4x6-8, Chest Dips 4x8-10, Pull-ups 4x6-8, BB Bent Over Row 4x8-10, DB Shoulder Press 3x10, Bicep/Tricep superset
  • Tue, Lower A (quad focus): BB Full Squat 4x6, Leg Press 4x10, RDL 4x8, Leg Curl 4x10, Hip Thrust 4x8, Calf/Abs superset
  • Thu, Upper B (volume/supersets): DB Incline Press 4x10, Cable Low Fly + Close Grip Lat Pulldown SS, Single Arm Row 3x10, Lateral Raise + Face Pull SS, Incline Curl + Pushdown SS
  • Fri, Lower B (ham/glute focus): Front Squat 4x10, Leg Extension 4x12, DB Lunge 3x12, Leg Curl 4x12, Cable Kickback 3x12, Seated Calf 4x20

Thursday 9 April - Rest Day Update (Wed 8 Apr)

Rest day yesterday. Pinned HCG 500IU and Puregon 75IU as per schedule.

Bumped the Exemestane to 12.5mg and the nipple sensitivity has settled down which is good. I have noticed some brain fog the last few days though so the increase was probably the right move, E2 was likely creeping up. Getting bloods done next week to confirm where things sit.

Big news on the IVF front, the clinic called to pre-book the start of the next implanting cycle for May. Still pending on the PGTA results from the last round, if those don't come through in time it'll be a new egg retrieval instead. Either way things are moving forward which is good.

Feeling really good today. Energy 9/10 after a massive 12 hour sleep (10pm to 10am, body needed it). Libido is solid at 8/10, woke up with morning wood which is always a good sign on this protocol. Mood 8/10 as well. Stuck to the meal plan yesterday no dramas.

Friday 10 April - Update (Upper B from Thu 9 Apr)

Upper B yesterday, volume day with three supersets. Hit all 8 exercises, 26 sets in about an hour. DB incline bench topped out at 40 per side for 8. Everything felt solid.

Protocol: No changes.
- Test E 40mg 2x/week (Mon, Thu)
- HCG 500IU 4x/week (Sun, Mon, Wed, Fri)
- Puregon 75IU 4x/week (Mon, Wed, Fri, Sun)
- Exemestane 12.5mg 2x/week

Fertility: Still waiting on PGTA results. Booking in for a semen analysis on Monday to see where the numbers are sitting now. Last one was 19.2M concentration with 48% motility so hoping to see some more improvement.

E2 management: Feeling good at the moment. No nipple sensitivity, no bloating, brain fog from earlier this week has cleared up. Exemestane at 12.5mg seems to be the right dose. Will confirm with bloods next week.

Sleep was about 9.5 hours (10pm to 7:30am). Body tried to wake me at 6 but I pushed through for another 1.5hrs. Energy 8/10, mood 9/10, libido 8/10. Meals on track.

Lower B yesterday, leg day with front squats, extensions, lunges, curls, kickbacks and calves. About an hour session.

Training:
- BB Front Squat: 15x10, 20x10, 30x10, 40x6 (per side)
- Lever Seated Leg Extension: 77x12, 90x12, 97x12, 104x12
- DB Lunge: 25x12, 30x12, 30x12 (per hand)
- Lever Seated Leg Curl: 70x12, 77x12, 84x12
- Cable Donkey Kickback: 18.1x12 x3 (each side)

Protocol: No changes.

Sunday 12 Apr Update (Sat 11 Apr)

Rest day yesterday between Lower B (Fri) and Upper A (Mon). Good recovery window before the week kicks off.

Meals: Mostly on plan, missed the afternoon oats. Smashed 3 hot dogs for dinner as a cheat meal, definitely felt it, didn't fall asleep til about midnight haha.

Protocol: Pinning HCG 500IU + Puregon 75IU today as per the standard Sun schedule. No other changes.

Today: Slept 8hrs great quality despite the late settle. Energy 7/10, Mood 7/10, Libido 7/10.

Sperm test done this morning. PGTA still pending, hopefully lands this week. Implanting cycle pre-booked for May if PGTA comes back clean, otherwise we pivot to a new egg retrieval.

Slept 11:30pm to 6:30am, a bit short. Was up early for the clinic, woke up with a mild headache. Energy 6/10, mood 6/10, libido 5/10. Nothing dramatic, just a flatter day.

Tuesday 14 Apr Update (Training update from Mon 13 Apr)

Mon is a double pin day on the fertility side, HCG 500IU and Puregon 75IU, both clean no PIP. Exemestane 12.5mg also went in Mon (bumped from 6.25 two weeks back after some nipple flare ups). Happy to report no nipple sensitivity or puffiness at the new dose, E2 feels like it's holding where we want it. Push on this dose a couple more weeks and then bloods to confirm.

Fertility side holding steady, no nipple sensitivity on Exemestane 2x 12.5mg weekly so E2 seems parked where it should be. Plugging along 💪
